What is Swing Bed Care?
Swing Bed Care is a short term, extended care program that you may be eligible for following hospitalization for an acute problem. If your condition has stabilized, but you are not ready to return home, you may be eligible for this program. By offering a combination of Acute Care and Swing Bed Care, Culbertson Memorial Hospital can see you through your illness and injury---from onset through recovery.
Your physician will discuss your specific illness, care, and length-of-stay in the Swing Bed Program. If needed, our Discharge Planner will help you make plans for long-term care.
Private funds, private insurance, and/or Medicare may pay for Swing Bed Care. Your physician and our hospital staff will help you determine if you qualify for Medicare coverage. If you meet the requirements, Medicare will pay 100 percent of approved charges for the first 20 days.
What are the Qualifications?
In order to be admitted to the Swing Bed Care program, you must meet the following Medicare guidelines:
What treatments are included in Swing Bed Care?

When you have surgery and need rehabilitation or skilled care, have your Discharge Planner contact Pam Tavernier, RN, BS, at Culbertson Memorial Hospital at: 217-322-4321, ext 295
By the way...
Hospitals call the program “Swing Bed” because the same beds used for hospital patients are used for Swing Bed Care, allowing patients to easily “swing” from one level of care to another.
